Graham lasted just 16 minutes in the Blues' 26-20 win in the decider at ANZ Stadium on Wednesday after limping off midway through the second-half.
The club said the 28-year-old had suffered a grade two hamstring tear and would miss up to a month, including Saturday's clash with Melbourne during which Storm skipper Cameron Smith will bring up his 400-game milestone.
Graham was walking freely in the Blues sheds afterwards but had an ice pack strapped to his hamstring.
While his absence is a big blow for the Sharks as they look to arrest a three-game losing streak, he has at least avoided a major injury.
Graham spent the off-season and the opening 12 rounds on the sidelines after suffering a torn ACL during last year's finals series.
He returned in round 13 and was rushed back into the NSW line up for game two where he put in an outstanding display filling in for halfback Nathan Cleary during the second-half in Perth.
Sharks coach John Morris is likely to start Kurt Capewell in the back-row against the Storm at AAMI Park.
